Specialist referrals commonly include the patient’s details, relevant clinical information, the referrer’s provider details and signature. Referral validity and Medicare requirements can change; the clinic must confirm current rules for each appointment.
Bulk billing and Medicare: Eligible Medicare services are bulk billed when Medicare requirements are met. Referral requirements and Medicare eligibility may apply. Please contact the clinic to confirm your eligibility and appointment requirements.
Urgent referrals: Clearly mark clinical urgency and contact the clinic directly. Online forms are not continuously monitored and must not be used for emergencies.
